The utilization of day surgery has witnessed a notable increase, extending its application to elderly patients due to the growing recognition of its benefits. Key factors driving the expansion of day surgery in elderly patients include the recent COVID-19 pandemic and the perception that the protocol can be safely implemented also in elderly individuals. The increasing demand for value-based healthcare has also accelerated this trend. Nonetheless, a lack of evidence regarding the safety of day surgery has impeded its development. Recent scientific evidence, however, suggests that the protocol is generally safe and feasible for many elderly patients, provided that underlying conditions that may affect its safe execution are thoroughly considered.
